Are you visiting one of our orthodontic offices for the first time?
We’re excited to welcome you and look forward to getting to know you better! Before your appointment, one of our Treatment Coordinators will call to introduce themselves, gather any missing information, and ensure we’re fully prepared to assist Dr. Podray-Donovan, Dr. Grochowska, or Dr. Lewis. This helps us ensure your consultation runs smoothly and efficiently.
When you arrive, you’ll meet with our Treatment Coordinator, followed by one of our wonderful doctors who will present a personalized treatment plan. After all of your questions are addressed, our fabulous Treatment Coordinators will take the time to review the treatment schedule and costs in more detail with you.
We leave the details of costs and insurance to our Treatment Coordinators so our doctors can focus on what they do best— straightening teeth!
Orthodontic Patient Forms
Before we can begin your appointment, we ask that you complete some paperwork, which includes your medical and dental history, dental insurance information, and more. To save you time in our waiting room, you can fill out these forms before you arrive. By taking care of this step in advance, our team can get you started right away.
Please note that a couple of signatures will be required inoffice for the HIPAA Privacy Policy. We will also need to collect your Photo ID and a copy of your Insurance Card. Having this information ensures we have the most accurate details for your insurance and helps any claims get processed promptly. Thank you for your understanding!
